Main Purpose of the Post To work with young people leaving the care system, in order to maximise their ability to live independent, responsible and fulfilling lives. To work towards achieving the 5 outcomes for Every Child Matters in partnership with Bradford Social Services. Functions 1. Referrals a) Receive and process referrals in line with current practice guidelines. b) To participate in identifying and assessing individuals' support needs, informed by relevant background information. Participate in the allocation of appropriate accommodation to those referred. Provide pre tenancy support where necessary. 2. Client Support a) Moving individuals into accommodation throughout the Project including organising practical tasks and benefit claims in line with current practice guidelines. b) To make regular support visits to residents in line with Care Leavers contracts. c) Providing support to residents, including help with budgeting, working on independent living skills and developing individual support plans.
